Orthodontic treatment is an excellent investment towards a "Million Dollar
Smile" and a "Healthy and Functional Bite." Financial considerations should
not be an obstacle to obtaining this important health service. Our office is
built on creating an "individualized" financial arrangement for our families.
We welcome the opportunity to help you include orthodontics in your budget.

Many orthodontic problems require only limited treatment at relatively
inexpensive fees. It is always our policy to discuss fees with you before
treatment begins.

As a courtesy to your family dentist, we provide a complimentary
new patient examination. Fees are generally determined by the severity
of the problem and the length of treatment time.

Payment options include:

Option I.

Payment in Full at Initial Treatment which will include a bookkeeping courtesy reduction.

Option II.

Office Payment Plan which involves an initial payment when treatment begins, with balance paid in monthly payments with the following options:

            A: Coupon payment booklet with pre-addressed payment
                envelopes
            B: Automatic Payment Plans ... for convenience, ease,
                and prevention of late charges:
                    1. Pre-Authorized Checking
                    2. Automatic Pre-Authorized Credit Card
